How to file a claim

Part II of the following claim forms will need completion by the insured's attending doctor/surgeon.

You are kindly reminded to make copy of your completed claim form and supporting documents for your record and future reference.

For medical expenses:
  • Passport, entry proof and travel tickets
  • Travel expenses receipts
  • Original medical receipt(s)
  • Medical reports from healthcare providers, or hospital discharge summary
  • Lab reports
  • Police report
  • Compensation breakdowns from other insurers or parties
For Compassionate Visit Benefit (ie. for sending one family member over for emergency):
  • Passport, entry proof and travel tickets
  • Travel expenses receipts
  • Medical reports from healthcare providers, or hospital discharge summary
  • Proof of relationship with the family member
  • Certificate of burial/cremation (if applicable)
For Personal Accident Benefit:
  • Passport, entry proof and travel tickets
  • Medical reports from healthcare providers, or hospital discharge summary
  • Police report
  • HKID of the insured and beneficiary (if applicable)
  • Death certificate (if applicable)
  • Certificate of burial/cremation (if applicable)
For journey curtailment (ie. the trip is interrupted) or cancellation:
  • Passport or entry proof
  • Unused ticket and deposit receipts
  • Written confirmation from the airline, transport company or hotel
  • Medical reports from healthcare providers
  • Police report
  • Death certificate (if applicable)
  • Certificate of burial/cremation (if applicable)
  • Compensation breakdowns from other insurers or parties
For loss of baggage, travel document, or personal money:
  • Passport or entry proof
  • Written confirmation from airline, transport company or hotel
  • Receipts for purchase of lost items or travel document replacement
  • Receipts for additional hotel accommodation and travel expenses 
  • Police report
  • Compensation breakdowns from other insurers or parties
For travel or baggage delay:
  • Passport or entry proof
  • Copy of boarding pass or air ticket
  • Written confirmation from airline, transport company or travel agency
  • Original receipts for the purchase of necessities, hotel accommodation and meals
  • Compensation breakdowns from other insurers or parties

Group accident

  • Sick leave certificate(s)

Group critical illness

Claim form
Required supporing documents
  • Confidential Medical Certificate, completed by the attending doctor
  • Copies of histopathology and lab results related to the illness
  • Certified true copy of HKID Card
Note:
  1. You MUST submit the claim through the policyholder (i.e. your employer) or the Human Resources Department of the policyholder.
  2. Certified true copy means a photocopy endorsed with the company chop and signature by the policyholder.

Group total & permanent disability

  • Pathological/medical reports related to the sickness
  • Sick leave certificate
  • Certified true copy of ID card
Note:

Group long term disability

  • Pathological or medical reports related to the disability 
  • Sick leave records
  • Certified true copy of ID card
Note:
  1. You MUST submit the claim through the policyholder (ie. your employer) or the Human Resources Department of the policyholder.
  2. Certified true copy means a photocopy endorsed with the company chop and signature by the policyholder.

Group death

Claim form
Required supporing documents
  • Certified true copy of Death Certificate
  • Proof of deregistration of ID card issued by the Immigration Department (if the deceased was a HKID cardholder and the death took place outside Hong Kong) 
  • Certified true copy of Notarial Certificate of Death, plus police report and hospital certificate (if the death took place in mainland China)
  • Sick leave certificate and attendance records of the deceased
  • Certified true copy of ID card of the beneficiary and the deceased
  • Certified true copy of the record of beneficiary designation, or Letter of Administration
Note:
  1. The death claim MUST be submitted through the policyholder (ie. the employer of the insured) or the Human Resources Department of the policyholder.
  2. Certified true copy means a photocopy endorsed with the company chop and signature by the policyholder.
