    Claim Document Checklist

    Required Documents

    • Copy of policyholder’s ID card (if not submitted previously)
    • For hospital & surgical benefit: original hospital or medical receipts and statements
    • For hospital cash benefit: copies of hospital receipts and statements of charges
    • Laboratory, ultrasound, X-ray, CT scan, MRI and pathological reports
    • Travel Document e.g. Passport / Entry Proof, Travel Tickets, Boarding Pass, etc.

     

    Additional Documents (as necessary)

    • Request for Return of Original Document(s)
    • Individual Life & Group Claims Arrangement
    • Discharge summary from government hospital (if applicable)
    • Sick leave certificate with diagnosis
    • Police and/or traffic accident report
